How many endings are there
Two. Both are confirmed by the achievement list on Steam. Some unofficial guides mention extra endings tied to Leviathan or hidden rituals, but neither full-game walkthroughs nor the achievement list confirm them — we do not describe things nobody has proven.

How the choice works
After the Ritual of Rebirth, Aidan returns to the apartment and the Cenobites make him a final offer. This is not a dialogue choice: what decides it is what you do with the character.
- The opening in the wall starts to close. If you walk through it after the Cenobites, you join them.
- If you stand still until the opening closes, you reject the offer and stay human. This is a deliberate decision, not a failure: you do not need to search for anything, attack anyone or solve another puzzle.
No earlier choice or collectible changes the result. There is also no "better" ending in terms of mechanics: neither gives a gameplay benefit, and each has its own achievement.
What happens to Sunny
In neither ending does Aidan get Sunny back. The difference is what he does after everything he has been through — not whether he managed to save her. The ending where he joins the Cenobites ends with Aidan's transformation; the game does not explain what his life in the order looks like afterwards, and there is no further chapter after it.
For some players that is a letdown: in Steam reviews the endings appear among the most common complaints ("the endings leave you wanting more"), although others praise the darker variant of joining the Cenobites.
How to see both endings without a second playthrough
The game creates a final autosave right before the decision, so you do not have to replay from the start:
- Reach the end of the story and the last scene in the apartment.
- Choose either ending and watch it to the end.
- Return to the main menu.
- Load the last autosave — you return to the apartment.
- Make the opposite decision and collect the second achievement.
The order does not matter. Worth remembering if you are collecting all achievements — full list: achievements in Hellraiser: Revival.
Just over 10% of players have each of these two achievements. That does not mean they are hard: most players simply have not finished the game yet — it launched on October 8, 2026.
